CREDIT APPLICATION – PLEASE NOTE: This must be completed. If you have a reference form you will be forwarding to us, we will still need the top portion of this form completed, less the trade reference information only. Please note also that by you verifying that you have correct phone numbers and fax numbers on your references, this will speed the process of your credit application.
TERMS OF CREDIT – This must be completed and signed by an officer in your company.
TEXAS SALES AND USE TAX EXEMPTION CERTIFICATE OR TEXAS RESALE CERTIFICATE Complete and return the one that would pertain to you.
